Entira Family Clinics is looking for a Registered Nurse Clinic Care Manager to be part of a dedicated team in a physician owned organization providing exceptional quality of care to our patients.
Responsibilities:
- Responsible for delivery of professional, quality and compassionate patient care.
- Assist providers in Patient Education.
- Carries out assigned nursing tasks and medical orders as delegated by clinicians to support patient care.
- Serves as a positive role model to clinical support staff.
- Partners with Care Coordinators to ensure appropriate support is available to the patients, families, clinicians and staff for service arrangements (home care, admission to hospitals), data tracking and other services as assigned.
- Provides clinical oversight to Care Coordinators as they support patients through care transitions such as home, hospital inpatient, nursing home, customized living and adult foster care and coordinates services for continuity of care and patient safety.
- Participates in care conferences with patients, families and clinicians.
- Participates in patients visits as requested for complex medical issues and/or education.
- Processes prescription refill requests promptly and accurately according to established protocols at patient visits.
- Provides education to patients/families related to clinical care.
- Dialogues with team members to ensure progress in meeting expected care and service outcomes.
- Assists team members in building strong relationships with clinic staff, clinicians and interdisciplinary team members involved in patient care.
- Serves as a resource to team members to enhance successful implementation of care plan.
Requirements:
- Current RN license in the state of Minnesota
- Graduate of an accredited RN program, Associate degree required, BSN preferred
- Previous RN experience in clinic system or care coordination preferred.

Entira Family Clinics is looking for a highly motivated, friendly, compassionate Registered Nurse looking to be part of a team dedicated to providing exceptional quality care to our patients.
Part-time benefits eligible, 32 hours per week.
RESPONSIBILITIES:
- Assess patient concerns/problems for all age groups over the phone through the collection of health data
- Plan/prioritize, implement and advise treatment based on approved protocols
- Completes prescription refills using approved company protocols
- Hospital follow-up calls
- Help develop and update written protocols
- Participates in special projects and committee work as requested
REQUIRED QUALIFICATIONS:
- Current RN license in the State of Minnesota
- Graduate of an accredited RN program
- 5 years RN experience in a variety of health care settings
- Previous triage experience preferred
- Experience with electronic medical records, eClinicalWorks experience preferred
- CPR certified
- Possess excellent oral, written and interpersonal communication skills with a high level of organizational ability and attention to detail
- Ability to collaborate with other members of the care team and able to work independently using effective nursing judgment and decision-making skills

Position Purpose:
The Certified Diabetes Education Specialist (CDES) is responsible for the education of patients, their families and appropriate support systems about diabetes self-management and related issues. The CDCES provides diabetes self-management education and coordinates protocol-based diagnostic testing and treatment, which may include medication management, nutrition, physical activity, healthy coping, risk reduction, monitoring and problem-solving. The CDES also serves as a resource to other staff members as needed. Provide consultation to providers on best practice in management of patients with diabetes.
Major Job Functions:
- Provides direct patient education in an individual or group setting following specific curriculum developed for diabetes education
- Assesses patient’s physical, psychosocial, behavioral and educational needs (to include self-management, nutrition, physical activity, medication taking, and health maintenance labs) and interprets the information to identify individualized clinical and educational needs
- Teaches technical stills, concepts of diabetes self-management and facilitates behavior change for patients with Type 1, Type 2, and gestational diabetes
- Collaborates with the patient to establish individual goals
- Communicates with providers, clinic staff and MTM pharmacists as part of the care team to keep the team aware of patient progress and recommends changes in treatment regimen as appropriate
- Evaluates the patient’s comprehension of completed education and develops follow-up plans to address ongoing needs
- Documentation in patient’s electronic medical record of required assessments, interventions, evaluations, and follow-up as consistent with established departmental and ADA standards
- Individualizes education presentation and materials to the patient and patient circumstance
- Utilize designated clinical guidelines and the ADA Standards of Medical Care in Diabetes for adult diabetes management
- Work with clinic management and staff to improve diabetes care and measures within the clinic
- Maintain ADA and/or AADE accreditation program records
- Required to travel to other facilities
- Perform other duties as assigned
Qualifications:
- Registered Dietician/Registered Dietician Nutritionist or Registered Nurse
- Current Certified Diabetes Care and Education Specialist (CDCES) certification or be in the process of completing Certification
- Current licensure in Minnesota
- Bachelor’s Degree preferred
- BLS Certified
- Experience with EMR
- Strong computer skills

JOB SUMMARY:
Provides quality expertise on use of standard methodologies, tools, and data collection and analysis to measure and evaluate quality projects, programs and other activities. Provides consultative support to clinics to facilitate improvement, which includes working with leadership, providers and clinic managers and staff to develop quality aims, measures, and strategies to improve performance. Provides expertise on best practice, and helps to ensure alignment of improvement activities and measures with medical practice guidelines and/or other resources. Provides quality improvement expertise to the organization by guiding, coordinating and facilitating and partnering with providers in clinical practice committees, task forces, and work groups, as assigned.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Coordination of assigned clinical practice committees including working with Committee Chairs to prepare agendas for meetings, ensuring that packets are complete, and helping to facilitate meetings.
- Uses quality improvement methods, tools and project management theory and techniques to develop and maintain project support for leadership and teams as assigned.
- Develops and maintains project documentation, work plans, and goals as assigned.
- Tracks follow up and action items generated from committees to ensure completion, communication and/or hand off.
- Works directly with other clinical practice team members regarding quality measures, reporting and audits.
- Works with, providers, managers and staff to identify opportunities for improvement by analyzing available data and information.
- Researches guidelines and available online resources to obtain clinical information that support best practice.
- Participates in tracking, trending and analyzing data related to quality measures.
- Works with medical director to investigate and follow up on quality of care concerns.
- Works with medical director to investigate and follow up on safety/risk management issues
- Works as the organizations Patient Safety Rep reviewing and analyzing patient safety events with other members of the care team.
- Coordinates health plan site visits and provides orientation to the EMR for auditors.
- Communicates routinely with the Manager of Quality providing information on:
- New and/or outstanding issues and concerns—internal and external
- Patient complaints, patient care concerns, or safety/risk management issues.
- Progress/status on P4P due dates and reports, grants and/or applications
- Changes in guidelines, requirements, regulations, etc.
- Trends, patterns, accuracy of quality measures
- Other quality related information essential for the department.
- Serves as advisor/QI resource to other committees or departments as needed.
- Serves as a representative of Entira Family Clinics with outside agencies, payers, vendors, other healthcare organizations, and the QI community.
- Actively pursues professional development to continuously maintain and enhance knowledge, skills and expertise.
- Promotes and supports data integrity and reporting timeliness.
- Creates and completes quality studies as assigned.
- Works directly with clinical practice staff to ensure accuracy of reports, data, measures, etc.
EDUCATION AND EXPERIENCE:
- Healthcare Background with Bachelor Degree (Masters preferred)
- Five or more years of experience in healthcare (clinic and/or health plan/payor experience preferred)
QUALIFICATIONS AND SKILLS:
- Knowledge and experience in ambulatory healthcare Quality Improvement
- Experience leading/facilitating and training QI activities
- Experience defining data needs and manipulating clinical data (Excel – Intermediate/Expert experience)
- Proficiency with Microsoft suite (e.g., Word, Excel, PowerPoint, Visio, etc.)
- Experience and training in quality methodology
- Demonstrated proficiency and analytical skills using computer systems
- Knowledge of quality standards/guidelines and regulations
- Project management training and experience
