We are currently seeking a Front Desk / Office Float to join our team. This is a Full-time, on-site role. This role is the back-up for all business office roles.

The expected base pay range for this position will be from $17.75 to $26.50, plus core and optional employee benefits. Entira Family Clinics considers various factors in determining actual compensation for this position. Actual compensation will be commensurate with demonstrable level of experience and training, pertinent education including licensure and certifications, and other relevant business or organizational needs.

Locations: Vadnais Heights

Job Functions:

  • Greet patients, collect insurance information and co-pays.
  • Orientation of new patients to clinic including distributing welcome packages
  • Scheduling outside appointments for patients with specialists, radiology, etc.
  • Completing the referral module for all referrals (specialist, outside tests and radiology and faxing to the insurance company as needed
  • Transmitting diagnostic imaging and physical therapy orders.
  • Entering orders into the Minnesota Gastroenterology web site
  • Creating orders for tests as needed
  • Sending appropriate notes, labs, and reports to specialists
  • Documenting dates, time, and place for specialist appointments
  • Performing quality checks to verify outside test appointments are kept.
  • Responsible for updating patient accounts, new patient registration, and taking billing calls.
  • Review accounts on future schedules.
  • Basic understanding of ICD-10 and CPT codes.
  • Post patient payments and adjustments.
  • Balance all transactions
  • Respond to inquiries regarding accounts from patients and insurance companies.
  • Prepare bank deposit forms.
  • Obtain patient medical and insurance information in order to coordinate the patient referral process.
  • Maintain strict confidentiality with patient information.
  • Participation with patient and family centered principles that are aligned with health care Home Standards

QUALIFICATIONS

  • High school diploma, GED or equivalent
  • One year experience in a medical office
  • Desired skills include but are not limited to: the ability to operate a PC system and 10 key calculator and understanding basic mathematical principles.
  • Must possess excellent communication, problem solving, and human relations skills.
  • This position requires the ability to deal with all types of people, the ability to be flexible and work in a fast-paced environment.
  • Knowledge of medical terminology.
  • Working knowledge of Excel.

Entira Family Clinics is looking for a Medical Assistant to be part of a dedicated team in a physician owned organization providing exceptional quality of care to our patients.

The expected hourly pay range for this position will be from $20.00 to $30.00, plus core and optional employee benefits. Responsibilities:

We are currently seeking a Care Coordinator/Patient Advocate to join our team. This is a full-time role and will be responsible for coordinating patient care.

Responsibilities:

  • Enrolls patient in CCM, (chronic care management) and coordinates patient care.
  • Use effective engagement strategies to build a trusting relationship with patients and families for ongoing partnership in their health care decisions.
  • Use a variety of educational materials, self-management support techniques, and community resources to engage patients, increase their motivation to change, and support patients and families in establishing healthy lifestyle goals and implementing treatment plans to meet those goals.
  • Develop a maintenance plan with patients to help them maintain a healthy lifestyle.
  • Contacts patients frequently (according to protocol) and arranges appointments with primary care provider (PCP) and specialists as needed.
  • Coordinates and facilitates communication between patient, families, primary care provider (PCP) and consulting specialists.
  • Supports patient with treatment compliance.
  • Assists patient when barriers to care arise.
  • Collaborates with PCP on outcome assessment using care plans.
  • Reviews caseload with PCP, focusing on patients not adequately improved within specified timeframe.
  • Assists patient in scheduling appointment with other health care providers and coordinates with the referral coordinator.
  • Documents all encounters in ECW according to Entira Family Clinics policies and procedures.

Requirements:

  • At least 1 year experience as Care Coordinator in clinical setting.
  • Associate degree or equivalent experience. CMA, MA, psychology, social work, health education or clinical background preferred.
  • Ability to manage a project from start to finish.
  • Ability to learn quality improvement principles/tools and methods.
  • Demonstrated ability to effectively establish and maintain collaborative working relationships with peers and constituents at all levels of an organization.
  • Knowledge of medical terminology.
  • Good desktop tool usage including Word, Excel, PowerPoint, and EMR.

The expected annualized base pay range for this position will be from $18.00 to $27.00, plus core and optional employee benefits. Actual compensation will be commensurate with demonstrable level of experience and training, pertinent education including licensure and certifications, and other relevant business or organizational needs. 

This position will require rotating weekends (roughly every 4-6 weeks). Potential for remote/hybrid work after 90 days.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Ensures telephones are courteously and professionally answered.
  • Receives and handles all telephone calls, resource, and routes to appropriate department.
  • Schedules patient appointments via telephone and patient portal.
  • Contact/reschedule patient appointments due to provider schedule changes.
  • Perform visit planning duties (create alerts/recalls, contacting patients for follow-up care).
  • Create provider mater schedule and review/edit daily schedules for each physician, as designated.

JOB SUMMARY:

Provides quality expertise on use of standard methodologies, tools, and data collection and analysis to measure and evaluate quality projects, programs and other activities. Provides consultative support to clinics to facilitate improvement, which includes working with leadership, providers and clinic managers and staff to develop quality aims, measures, and strategies to improve performance. Provides expertise on best practice, and helps to ensure alignment of improvement activities and measures with medical practice guidelines and/or other resources. Provides quality improvement expertise to the organization by guiding, coordinating and facilitating and partnering with providers in clinical practice committees, task forces, and work groups, as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Coordination of assigned clinical practice committees including working with Committee Chairs to prepare agendas for meetings, ensuring that packets are complete, and helping to facilitate meetings.
  • Uses quality improvement methods, tools and project management theory and techniques to develop and maintain project support for leadership and teams as assigned.
  • Develops and maintains project documentation, work plans, and goals as assigned.
  • Tracks follow up and action items generated from committees to ensure completion, communication and/or hand off.
  • Works directly with other clinical practice team members regarding quality measures, reporting and audits.
  • Works with, providers, managers and staff to identify opportunities for improvement by analyzing available data and information.
  • Researches guidelines and available online resources to obtain clinical information that support best practice.

  • Serves as advisor/QI resource to other committees or departments as needed.
  • Serves as a representative of Entira Family Clinics with outside agencies, payers, vendors, other healthcare organizations, and the QI community.
  • Actively pursues professional development to continuously maintain and enhance knowledge, skills and expertise.

EDUCATION AND EXPERIENCE:

  • Healthcare Background with Bachelor Degree (Masters preferred)
  • Five or more years of experience in healthcare (clinic and/or health plan/payor experience preferred)

QUALIFICATIONS AND SKILLS:

  • Knowledge and experience in ambulatory healthcare Quality Improvement
  • Experience leading/facilitating and training QI activities
  • Experience defining data needs and manipulating clinical data (Excel – Intermediate/Expert experience)
