Entira Family Clinics is looking for a Medical Assistant to be part of a dedicated team in a physician owned organization providing exceptional quality of care to our patients.

The expected hourly pay range for this position will be from $20.00 to $30.00, plus core and optional employee benefits. Entira Family Clinics considers various factors in determining actual compensation for this position. Actual compensation will be commensurate with demonstrable level of experience and training, pertinent education including licensure and certifications, and other relevant business or organizational needs.

Hours: 7:00am - 4:00pm, Tuesday - Friday (32 hours per week)

Responsibilities:

  • Show patients to exam rooms and prepare them for the provider.
  • Obtain vitals and interview patients to obtain medical information.
  • Update patient’s electronic medical record
  • Prepare and administer medications as directed by a provider.
  • Collect samples and perform routine laboratory tests.
  • Operate x-ray, electrocardiogram (EKG), and other equipment to administer routine diagnostic tests.
  • Stock and prepare rooms.

Requirements:

  • Completion of an accredited Medical Assistant program
  • Limited scope x-ray operator’s license required
  • Ability to multi-task and work in a fast-paced environment

Entira Family Clinics is looking for a Registered Nurse Clinic Care Manager to be part of a dedicated team in a physician owned organization providing exceptional quality of care to our patients.

The expected hourly pay range for this position will be from $30.00 to $39.00, plus core and optional employee benefits. Entira Family Clinics considers various factors in determining actual compensation for this position. Actual compensation will be commensurate with demonstrable level of experience and training, pertinent education including licensure and certifications, and other relevant business or organizational needs.

Responsibilities:

  • Responsible for delivery of professional, quality and compassionate patient care.
  • Assist providers in Patient Education.
  • Carries out assigned nursing tasks and medical orders as delegated by clinicians to support patient care.
  • Serves as a positive role model to clinical support staff.
  • Partners with Care Coordinators to ensure appropriate support is available to the patients, families, clinicians and staff for service arrangements (home care, admission to hospitals), data tracking and other services as assigned.
  • Provides clinical oversight to Care Coordinators as they support patients through care transitions such as home, hospital inpatient, nursing home, customized living and adult foster care and coordinates services for continuity of care and patient safety.
  • Participates in care conferences with patients, families and clinicians.
  • Participates in patients visits as requested for complex medical issues and/or education.
  • Processes prescription refill requests promptly and accurately according to established protocols at patient visits.
  • Provides education to patients/families related to clinical care.
  • Dialogues with team members to ensure progress in meeting expected care and service outcomes.
  • Assists team members in building strong relationships with clinic staff, clinicians and interdisciplinary team members involved in patient care.
  • Serves as a resource to team members to enhance successful implementation of care plan.

Requirements:

  • Current RN license in the state of Minnesota
  • Graduate of an accredited RN program, Associate degree required, BSN preferred
  • Previous RN experience in clinic system or care coordination preferred.

We are currently seeking a Care Coordinator/Patient Advocate to join our team. This is a full-time role and will be responsible for coordinating patient care.

  • Enrolls patient in CCM (chronic care management) and coordinates patient care.
  • Use effective engagement strategies to build a trusting relationship with patients and families for ongoing partnership in their health care decisions.
  • Use a variety of educational materials, self-management support techniques, and community resources to engage patients, increase their motivation to change, and support patients and families in establishing healthy lifestyle goals and implementing treatment plans to meet those goals.
  • Develop a maintenance plan with patients to help them maintain a healthy lifestyle.
  • Contacts patients frequently (according to protocol) and arranges appointments with primary care provider (PCP) and specialists as needed.
  • Coordinates and facilitates communication between patient, families, primary care provider (PCP) and consulting specialists.
  • Supports patient with treatment compliance.
  • Assists patient when barriers to care arise.
  • Collaborates with PCP on outcome assessment using care plans.
  • Reviews caseload with PCP, focusing on patients not adequately improved within specified timeframe.
  • Assists patient in scheduling appointment with other health care providers and coordinates with the referral coordinator.
  • Documents all encounters in ECW according to Entira Family Clinics policies and procedures.
  • Helps maintains registries as directed.
  • Works with resources provided for care coordination and helps develop additional resources as needed.

Requirements:

  • At least 1 year experience as Care Coordinator in clinical setting.
  • Associate degree or equivalent experience. CMA, MA, psychology, social work, health education or clinical background preferred.
  • Ability to manage a project from start to finish.
  • Ability to learn quality improvement principles/tools and methods.
  • Demonstrated ability to effectively establish and maintain collaborative working relationships with peers and constituents at all levels of an organization.
  • Knowledge of medical terminology.
  • Good desktop tool usage including Word, Excel, PowerPoint, and EMR.
  • Excellent communication skills, both verbal and written.
  • Demonstrated analysis and problem solving skills.
